We are very excited to announce that we have recruited our first three PCPs. These new professionals come from backgrounds in Community Nursing or Paramedic or Physician Associate. They are working with the GPs and Community nurses ( District nurses and others) to further improve the work we do with our patients in Care Homes and in peoples' own homes. Helping with home visits when someone suddenly becomes unwell, reviewing patients with long term conditions like COPD or Diabetes who cannot get to the surgery, and supporting the care of people in Care Homes who need to have reviews of their overall health.
This is a new service but it is already making an impact and we hope to grow the number of people involved AND work closely with our District Nursing colleagues to develop really excellent community offer to all our patients who cannot get into the surgeries.
