The Care and Connect Team offer advice, guidance and information to enable individuals to stay independent and connected with their community. The team offers detailed knowledge of the local care & support system alongside information, and where needed, access to the Council's adult social care system. Care and connect aims to connect you to services that can reduce Isolation, maintain and improve health & wellbeing and encourage community access. Guidance is provided by telephone or face to face in community settings. We can connect you to activities; services; groups; organisations and events that support wider wellbeing in North Tyneside.
